WAIVER OF LIABILTY:
My existing eyelashes, and that notwithstanding the utmost of care in the application or removal of these products, there still exist risks associated with the procedure and product itself, which may include eye irritation, eye pain, discomfort, and, in rare cases, blindness even when applied in the usual manner. If I (client) experience any irritation, redness, puffiness, itchiness, an allergic reaction or any other side effect of this procedure, I will contact my; a lash tech to remove or medical doctor immediately. As part of this procedure, I understand that a certain amount of eyelash adhesive material will be used to attach the artificial eyelashes to my existing eyelashes. Even though the eyelash extension artist may apply or remove my eyelash extensions in the usual manner, I understand adhesive material may become dislodged during or after the procedure, which may irritate my eyes or require further follow-up care, at my own expense to prevent damage to my eyes. I also understand there is more than one technique for applying eyelash extensions to my eyelashes, and I will not attribute any liability to the eyelash extension artist as a result of this procedure or the use and care of these lashes. As part of the removal procedure, I understand that a certain amount of chemical adhesive remover is applied to existing adhesives and a reaction occurs to dissolve the adhesive that results in the thinning of the remover. Even though the eyelash extension artist may apply or remove my eyelash extensions in the usual manner, I understand the liquid remover may seep into my eyes, which may irritate my eyes or require further follow-up care, at my own expense to prevent damage to my eyes. I also agree to defend, indemnify and hold harmless the eyelash extension artist from any and all claims, actions, expenses, damages and liabilities, including reasonable attorneys’ fees which might be asserted against her as a result of my having this procedure performed, or my purchase of these eyelash extension products from her. Please be aware rubbing, tugging or pulling on your lashes to remove them, apply lashes after can cause fracture to your natural lashes, swelling, pain, redness, and even cause permanent damage to the eyelash follicle. Delzora is not liable for any medical issues. I (client) understand I would need to contact Delzora for lash removal within 24 hours after service otherwise I will need to book a lash removal.
BEFORE APPOINTMENT:
Come with face clean, use restroom before scheduled appointment, NO EXTRA GUESTS IF YOU ARE NOT BEING SERVICED (including children's) I DO NOT WORK ON OTHER TECHS WORK. DO NOT GLUE YOUR LASHES ON YOURSELF I WILL HAVE TO REMOVAL AND DO A NEW SET OR YOUR APPOINTMENT WILL SERVICED AS A FOREIGN FILL. IF NOT ENOUGH TIME I WILL THEN HAVE TO CANCEL YOUR APPOINTMENT. See cancellation policy. COME WITH FACE CLEAN OR BOOK LASH REMOVAL SERVICE. Same day or squeeze in appointments is a fee of $35.
RESCHEDULE/CANCELLATON POLICY:
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the eyelash extension artist's day that could have been filled by another client. As such, we require 8 hours' notice for any cancellations or reschedules. Clients that provide less than 8 hours' notice will be charged a last minute inconvenience fee of $35 cancellation or reschedule. 1-4 hours before appointment result in payment of remaining balance. Your next two appointments will be full payment.
TARDINESS:
PLEASE ARRIVE ON TIME! You have a 5-minute grace period. After, late fee of $15 will apply. After 15-minutes your appointment will be cancelled, inconvenience fee will be charge card on file or invoice will be sent to your email and you will be banned until payment is completed. You will then need to book another appointment (another deposit is required.) Next two appointments will be full payment.
NO SHOW POLICY:
There will be a value fee of $50 if client does not show to their appointment. Next two appointments will be full payment.
THIEVERY:
Shop has 24 hours surveillance. Any belongings that are Delzora Rose taken out of shop without permission will be a theft fee of $100; can no longer book again as well as anyone who is associated with client. If service provider feels as though belongings was taken accidentally then return to service provider immediately.
BANNED POLICY:
A client might be banned from a platform or service for violating the service provider guidelines or terms of service policies, which could include activities like taking advantage of services or service provider, harassment, sharing personal information, posting inappropriate content, being problematic, using someone else information to book, engaging in fraudulent behavior, or repeatedly breaking the rules or policies.
PERMISSIONS TO USE PHOTOGRAPHS:
I hereby grant the eyelash extension artist the full right to take, publish and reproduce photographs of me, my face, my eyes and/or eyelashes, both before and after this procedure, for any advertising, education, or other purposes whatsoever, including the right to retouch these photographs as deemed necessary by the eyelash extension artist. I further expressly assign any copyright in these photographs to the eyelash extension artist. I also grant my consent for the eyelash extension artist to use my image and likeness as contained in these photographs for any advertising or other purposes.
CARE AND MAINTENANCE:
I agree to follow the care and maintenance instructions provided by the eyelash extension artist for the use and care of my eyelash extensions, and that if any follow up care is required due to my own mistake or negligence, or failure to follow these instructions, this will be at my own expense and risk. I understand that if I do any of the following, it may result in damage to my eyelash extensions or may cause my lashes to fall off prematurely. Knowing this I agree to follow these tips for best results:
- I will avoid touching the lashes or spending any prolonged amount of time with the lashes wet for the first 24 hours to protect my lashes.
- I will avoid oil-based eye products, as these will loosen the bond of my eyelash extensions.
- If I experience any itching or irritation, I agree to contact the eyelash extension artist to have the eyelash extensions removed or a medical doctor immediately.
- I agree to avoid using mascara and to not use an eyelash curler, perm, or tint my eyelash extensions.
- I agree to not pick, pull or rub my eyelash extensions.
- I understand that I should not attempt to remove my lash extensions on my own or with any product,
but that the procedure requires that my eyelash extensions be professionally removed. I understand
that if I pick, pull on, or rub my eyelash extensions it may result in the premature temporary and/or
permanent loss of my artificial and natural eyelashes, swelling, pain, and redness.
- Sleep position.
NO KNOWN MEDICAL CONDITIONS/ INFORMED CONSENT:
I acknowledge that I have been advised of the potential harmful or negative side effects that the lash extension procedure or removal may cause to those who have specific medical or skin conditions. I understand that the adhesives and adhesive remover are a skin, eye and mucus membrane irritant and that in rare cases persons may be allergic or have hypersensitivity to synthetics, cyanoacrylate or formaldehyde, which in small amount may be present in the adhesive. I understand that the procedure requires that I lay still for up to 2 hours or longer with my eyes shut, and that if I wear contacts, I must remove my contact lenses for the duration of the lash extension application or removal. I further state that I have no known medical condition that might be aggravated by the procedure or any medical condition that would prevent me from complying with or heeding to the eyelash extension artist's instructions or these warnings.
I understand that I will receive reminder text messages and emails for my appointment. I understand I will be charged fees for last minute cancellations, reschedules, and/or no shows. I understand that I will receive email confirmation of continued policies.
All services are non-refundable after day of appointment. Fees are applied if client did not select their add on before booking. Deposit non-refundable
SPECIALS, PROMOTIONS, DEALS, GIFTS:
All specials, deals or gifts is final sale and requires full payment. Due to limited slots special is non-refundable, non-transferable and do not be combine with any other coupons. Make sure you add on any additional services because there’s no guaranteed service if it’s busy for that day. Rescheduling appointments may no guarantees of future slots which means you will loose your appointment.

Loyalty reward policies, terms and conditions:

Members must maintain an active membership to earn and redeem points.

Customers can opt out of the program at any time by contacting customer service, and will forfeit any unused points.

Not transferable to another person: meaning that you cannot share your loyalty points with other people.

Not exchangeable for cash: meaning that the earned points or rewards cannot be exchanged for their monetary value.

Redemption limits: you only can redeem rewards once you get to reach amount of points.

Expiration dates: points have no monetary value and they will not expire if you make at least one purchase every few months. Points expire after 12 months if you do not make any purchase in the 12 months after earning them.

Purchase requirements: A minimum merchandise purchase of $5.01 (excluding taxes and shipping charges) is required in order to redeem your reward coupons.

Promotion stacking: You can only use one voucher at any one time when making a purchase or booking an appointment, and it cannot be combined with any other voucher, discounted codes, discounted items, deals or specials. The voucher will only apply to product or service prices and will not apply to any delivery costs

Redemption conditions: each reward may be redeemed for only one item and will be applied to the less or most expensive eligible product in the transaction. You must identify yourself as a Participant at the time of transaction to redeem the rewards.

Tiers specification: The tier level of your membership is based on the points earned during the current or previous Membership Year whichever is the highest.

Fraud prevention: We receive the right to cancel or suspend your participation in the Program or any Rewards earned and not yet used in the event of fraud, abuse of rewards privileges, violation of these Program or any Terms and Conditions, or at our discretion.

This is not a clothing store. You cannot return lashes. If you are having medical issues as swelling, redness, irritation during or 12hours after service inform me so I can remove your lashes. After, you will need to book for a lash removal. IF YOU RECENTLY PLUCK, RUB, PULL, ETC TO REMOVE YOUR LASHES OR HAVING BAD ALLERGIES CANCEL/RESCEDULE YOUR APPOINTMENT ASAP!!!!
For sensitive eyes or have allergies I recommend taking allergy medicine before appointment. Use eye drops and do not put your hands in your eyes
I AM NOT RESPONSIBLE FOR ANY MEDICAL ISSUES TO YOUR EYE. I CAN ONLY REMOVE YOUR LASHES WITHIN 12HOURS OF APPLYING LASHES. AFTER, YOU WILL NEED TO BOOK A LASH REMOVAL. LASHES EXTENSIONS CAN NOT BE PLACED UNTIL 1/2 WEEKS AFTER REMOVAL.

• After missed appointments, you will need to pay full total of service booked for your next two appointments

Plan your transportation before your appointment. Fees will be added to service after grace period. If there's not medical, family, or emergency from the state which held you from your appointment, you will still be charged full payment+ inconvenience fee if you are passed your grace period.

If your session hits between 6am-9am or 5pm-8pm is considered $35 fee

• All guests are required to have a valid card on file
• If card declines, the account will be blocked until card is updated and processed.
TARDINESS POLICY:
• If you are running late, please notify me as soon as possible.
• A 10-minute grace period is allowed for all guest
CANCELLATION POLICY:
• Please cancel appointment 8 hours in advance. Cancellations with less than an 8-hour notice will be charged at 50% service value. Cancel/reschedule 1-4 hours before appointment will be charged full service including $15 inconvenience fee.
